If you require any vaccinations relating to foreign travel you will need to make an appointment with the Practice nurses, but first you will need to complete a questionaire (see below or collect from reception).
This questionaire asks you to advise which countries and areas within countries that you are visiting to determine which vaccinations are required. The nurse will then review your needs and call you to advise what is recommended, and where appropriate arrange an appointment.
Please note that many vaccinations are not provided by the NHS and accordingly a fee may be payable. The nurse will advise you about this.

If you are travelling abroad please complete and submit the travel form in plenty of time (we recommend 6-8 weeks) before your journey.
