Patient must meet the following criteria:
- 18+ years old
- BMI of 30 and above
- BMI of 27.5 and above if BAME
- Has diabetes, hypertension or both
Exclusions:
- Moderate to severely frail
- Pregnant
- Has had bariatric surgery within last 2 years.
