Patient Registration
To register with the practice you must live within the practice area and bring in your medical card or fill in a registration form at Reception. You will also need to bring some form of identification - preferably one item of photographic id and another showing your current addess.
When you register as a new patient it may take some months before we receive your medical record. This is because we have to request it from your previous practice via the Northamptonshire Primary Care Trust. We therefore ask you to complete a short questionnaire alongside the standard registration form.
